1)  Most will see increases of $2 to $5 in out of pocket expenses.

2)  American Express Cards will no longer be accepted.

 CREDIT CARDS, AND DEBIT CARDS USED WITHOUT A PIN# WILL

 INCUR A 3.5% SERVICE FEE. 

HSA CARDS WILL ONLY BE ACCEPTED AS A DEBIT CARD WITH A PIN#. CONTACT YOUR CARD CARRIER FOR PIN#.

3)  Dr. Durkee participates in BCBS (PPO), UHC (PPO) & Medicare(Traditional - No Managed Care) and will continue to send claims electronically on your behalf - HOWEVER, allowed amounts may be collected at the time of service. As your deductibles are met, your insurance carrier should send payment directly to you. Medicare will not have changes to the billing procedures.
