Right to restrict disclosures -

You have the right to restrict disclosure of PHI (Personal Health Information) to health plans under certain situations.  If you wish to add that layer of protection, you must have a signed document stating your intent on file which must include option to pay-in-full at the time of service and insurance companies will not be billed.

We create a record of care and services you receive at our clinic. We need this record to provide quality care and comply with certain legal requirements.
