Appointment & Credit Card Policies
Dr. Ruth Durkee has been in private practice since 1995 and is starting a new chapter. She is now semi-retired and will continue to see her patients by appointment only.

It is important to arrive on time for appointments as she attempts to see her patients on time.
​
Please note: There is a 3% service fee for use of credit cards. A debit card used with a PIN# will not incur a fee. An HSA Card requires a PIN#, contact your card carrier.

There is a late cancellation or no-show fee of $25.00. Please give the office 24 hours notice of any appointment changes. This allows us time to schedule someone on a cancellation list.

Right to restrict disclosures -
You have the right to restrict disclosure of PHI (Personal Health Information) to health plans under certain situations. If you wish to add that layer of protection, you must have a signed document stating your intent on file which must include option to pay-in-full at the time of service and insurance companies will not be billed.
